OREANDA-NEWS. May 15, 2014. The commission which was created on the order of Siberian Administration of Russian Technical Supervision (Rostechnadzor) for technical investigation of causes of the accident completed its work on the enterprise; the commission investigated the accident which took place on 6th March in the phenol-acetone workshop of the plant “Omsky Kauchuk”.

According to the act of official investigation, technical cause for the accident was depressurization of isopropyl benzene pipeline after external mechanical exposure. Further inflammation of the product took place after a spark hit the depressurization area; the spark was caused by flame works.

On the enterprise one revealed deficiencies in organization of repair works and instrumental equipment of control systems and anti-malfunction protection.

The commission determined that employees of contracting organizations who conducted repair works on the plant “Omsky Kauchuk” industrial site, and workers of the enterprise made a number of violations of the RF legislation in the sphere of industrial and fire safety. Members of the commission concluded, that technological regime of phenol-acetone production was observed in accordance with regulating parameters. One analyzed fullness and periodicity of conduct of repair works on technological equipment; repair logs and also equipment run logs, instruction on organization and safe execution of repair works were checked. One detected no violations of order and terms of execution of equipment repairs.

Let us mention, that as a consequence of an accident four out of six oxidation columns were put offline in the phenol-acetone workshop of the “Omsky Kauchuk” plant. Ten persons were recognized as casualties, two of them were hospitalized. To date they were discharged from medical institutions.

After the results of the investigation public company “Omsky Kauchuk” and the enterprise’s personnel were drawn to responsibility under administrative law. Nowadays Investigations Direction of Investigations Committee of the Russian Federation in Omsk Region continues to work on the plant, one conducts an unplanned check of the Siberian Administration of Russian Technical Supervision.
